When the medical record tells a different story — you need someone who can read every line.

I’m Dr. Melissa Skoff, DNP, AGACNP-BC, FNP-BC — a dual board-certified Nurse Practitioner with advanced training in cardiovascular surgery, critical care, and acute care medicine. I bring clinical precision and legal fluency to every case, helping attorneys understand not just what happened — but what should have happened, and why it matters.

Most legal nurse consultants are RNs. I’m not most legal nurse consultants.

Why Attorneys Work With Me

The difference between winning and losing a complex medical case often comes down to one question: Do you have the right clinical expert at the table?

As a DNP-prepared Nurse Practitioner with hands-on experience in cardiovascular surgical ICUs, acute care, and emergency medicine, I don’t just summarize records — I understand the clinical reasoning, the documentation standards, and the decision points that determine whether care met or missed the mark. I’ve managed the most complex patients in the highest-acuity settings. I know what good care looks like. And I know when it’s missing.

Whether you’re building a plaintiff case, mounting a defense, or evaluating a case for settlement, I give you the clinical clarity to move forward with confidence.

Medical Record Review & Analysis

A thorough, organized review of the full medical record with attention to clinical accuracy, documentation gaps, and deviations from accepted standards. You receive a clear, attorney-ready summary identifying the key clinical events relevant to your case theory.

Standards of Care Analysis

I evaluate whether the care provided aligned with established clinical standards, guidelines, and reasonable professional expectations. This includes nursing standards, advanced practice provider (NP/PA) standards, and physician-ordered care as documented.

Best for: Cases involving alleged nursing negligence, NP/PA scope-of-practice questions, deviation from clinical protocols

Clinical Chronology Development

A precise, timeline-based reconstruction of clinical events drawn directly from the medical record. Organized to support deposition preparation, expert witness briefing, and trial narrative development. I translate complex medical sequences into a format your entire legal team can use.

Best for: Cases with high record volume, multi-provider involvement, or time-sensitive causation questions

Doctor Reviewing Documents

Before committing resources to a complex medical case, let me tell you whether the clinical record supports your theory. I provide a focused, efficient early-stage review to identify whether a case has merit — saving you time and litigation costs upfront.

Best for: Plaintiff and defense firms evaluating intake cases, insurance defense and risk management teams

Healthcare Professional Defense Support

Are you defending a nurse, nurse practitioner, or physician assistant in a malpractice action or licensing board proceeding? I offer specialized support rooted in direct clinical experience as an NP myself.

I understand NP scope of practice, clinical documentation expectations, and the standard of care for advanced practice providers — from the inside. I can help you evaluate whether the provider’s clinical reasoning and actions were consistent with sound advanced practice standards, and I can help you explain that clearly to a judge, jury, or licensing board.

Best for: Defense attorneys representing NPs, PAs, and RNs; attorneys handling licensing board defense matters
